What is pancreatic cancer?

Pancreatic cancer is an aggressive malignant tumor that affects the pancreas - an organ that produces digestive enzymes and hormones. А tumor usually occurs in the head of the pancreas (in 50-60% of cases), in its body (10%) or tail (5-8%) and spreads to the liver, peritoneal cavity, and lungs.

There are 20 types of pancreatic tumors. The most common, pancreatic ductal adenocarcinoma, accounts for about 85% of cases. Less common is neuroendocrine cancer (only 2%).

My 6-year old son was diagnosed with epilepsy. We couldn’t find an appropriate specialist in Czech Republic who could adequately assign medical treatment for 6 months. So, my wife and I decided to go for diagnostics and treatment abroad. The first sensation of my arrival - I am very satisfied with the service. We were greeted, brought to the hospital and what was promised to us was all done. We have a phone and when we do not understand something, we call and our translator immediately comes. I like it very much. And we have raised the hope that doctors here can help us and I really believe in that. We have already made PET, MRI, blood tests and now passing the fourth day of video monitoring. We asked Bookimed to find a clinic that can help us, then we had all our data sent and we were offered Spain. They said it is one of the best clinics in general.

Symptoms and diagnostics of pancreatic cancer

There are no symptoms in the early stages. Even in case of metastasis they are non-specific. Advanced stages can cause lack of appetite and weight loss.

Other symptoms of pancreatic cancer in the later stages:

  • severe pain in the upper abdomen;
  • nausea, vomiting;
  • urine darkening;
  • jaundice (yellowing of eyes and skin);
  • diarrhea or constipation;
  • apathy, fatigue.

The disease is often found at advanced stages because early signs are similar to other conditions like gastritis and gallstones.

If you have these symptoms - visit your doctor as soon as possible. It is important to undergo all test that are needed for diagnosis.

Pancreatic cancer is aggressive, rapidly spreads and affects the vital organs of the gastrointestinal tract. In the early stages of the disease, surgical intervention is the most effective treatment.

But only 25% of all patients can be operated due to the complex anatomy of the organ and vast metastasis.

The type of surgery depends on the pancreatic lesion:

  • Pancreatectomy (surgical removal of the pancreas). It is performed if cancer affected body and tail of the organ. It can be:
    - distal pancreatectomy (removal of the bottom half of the pancreas);
    - segmental pancreatectomy (resection of the middle part of the pancreas);
    - total pancreatectomy (entire pancreas).
  • Whipple procedure (pancreaticoduodenectomy). It is performed if cancer affected the head of the pancreas. A surgeon removes pancreas along with the gallbladder, duodenum, parts of the bile duct and stomach.
  • Pancreaticoduodenectomy with extended retroperitoneal lymphadenectomy - Whipple procedure with additional lymph nodes removal.

After resection, almost 20% of patients need the 2nd operation to restore the connection between the stomach and duodenum.

NanoKnife for pancreatic cancer

NanoKnife is a form of ablation used for localized pancreatic cancer. It can be performed during surgical intervention or as a stand-alone procedure. NanoKnife kills cancer cells with an electric current without the risk of damage to organs and blood vessels. Advanced stages treatment options

Palliative therapy

In case of last stages, physicians use palliative methods, which can significantly improve and prolong the life of a patient, including:

  • surgery;
  • prevention of gastric obstruction;
  • pain relief;

Despite us thoroughly pre-screening several hospitals and all looking good on paper and the website of this one our experience locally was just terrible! Waiting in the queue with dozens of other customers for an hour just to be registered... Interpreter who we did not order but had to pay for her services. However, the climax of all the process was the diagnostics and consultation with the doctor for which we paid 550Euro and then had to wait for nearly 10 days for the official report when it was promised to be delivered to us next day! Local coordinator Alexandra Zinnemann despite being the focal point just moved away from solving any issues blaming her other colleagues! Stay away!!! Not going to recommend to any of my friends in the future this clinics!

I worked with bookimed to explore options for brain tumor surgery in Turkey. In less than 24 hours after filling out the form online with basic information, received a call from bookimed specialist. It was very helpful that Ivan Koval spoke both Russian and English. Upon sending patient's scans, Ivan was able to give his feedback on urgency of the matter and relayed all of that in both languages. Within one day, we received responses from 2 clinics. With Ivan's help we were able to evaluate both bids as well as quality of facilities. It was very reassuring that Ivan knew the names of the specialists that I had already researched. Since the parents of the child insisted that I made at least one contact directly with the clinic, the delay of the direct response and the quality of the bid were very disconcerting. Direct contact with a clinic was done by the clinic's administrative staff who began making contacts with orthopedist and ophthalmologist (based on symptoms caused by the brain tumor!). Contrary to the parents' expectations, direct contact was not "more economic" but turned out to be too vague and still unclear if the correct department was ever consulted. Ivan's situation was very difficult as he had to deal with a family who made decisions based on the cost (no regard for quality) and word of the mouth (no differentiation as to which procedure was done). He was very patient and compassionate to the fate of the child. Ivan was able to relay to the child's parents diagnosis, bids, comparisons, and complexity of the procedure to be performed. It takes several hours per day and several days to coordinate the correct diagnosis based on scans and numerous phone calls between different time zones to coordinate quick and accurate response. I received updates and calls as soon as they came in which implies that Ivan worked 24/7 to make the best arrangements. While my initial contact was a shot in the dark, over the course of few days I began to appreciate the sense of confidence that comes from having a trained medical specialist to represent your interests when "shopping" for a brain surgeon. No amount of googling and online reviews can substitute opinion from a trained medic in dealing with various clinics. At the end of the day, I feel that having Ivan to oversee the process from admission to the RIGHT clinic to take care of the RIGHT procedure (brain and not foot) all the way to discharge is a solid form of medical insurance.

I took psychiatric consultation from Dr Achal Bhagat. Had a miserable experience. He asked me to visit his Clinic in Safdurjung for counselling. He put me on to a trainee who was pathetic and would just sit there humiliating me. When I shared my concern with him, he abused me and said that I need 40 more therapy sessions. He put me onto anti depressant when I didn't even need it and this medicine completely messed up my system. It's really sad that a hospital like Appolo has such doctors. We would never visit this hospital.
