Indications and contraindications for bone marrow transplant

Indication for transplantation of bone marrow is achieving a condition in which own blood-forming tissue can not cope with its function. In non-cancer diseases it is the lack of production of stem cells, in leukosis it is their production in “immature” state.

Contraindications to the procedure are:

  • Acute infectious diseases (hepatitis B and the C, syphilis, HIV).
  • Diseases of the immune system.
  • Severe diseases of internal organs.
  • Pregnancy.
  • Elderly age.
  • Long-term taking of hormonal drugs or antibiotics.

Types of bone marrow transplantation in Turkey

Transplant surgeries are divided according to the source of producing hematopoietic material for transplantation into:

- Autologous (transplantation of own hematopoietic cells, taken earlier).

- Homogeneous (material is transplanted from another person):

  • related – of a close relative. A particular case of this type of transplant is syngenesiotransplantation, where the material of the twin is transplanted.
  • unrelated –of an unrelated donor.

Autologous transplant is conducted to the patients with malignant tumors if the cancer has not affected bone marrow tissue or disease went into remission. It is important to conduct the transplantation before the tumor becomes resistant to treatment. This type of transplant does not create compatibility problems, which becomes the main when it is allogeneic.

Transplanted bone marrow must genetically match the tissues of the recipient. Otherwise there is a risk of complications of two types:

  • Graft-versus-host when the transplanted cells perceive the recipient’s cells as “alien” and begin to attack them. The occurrence of such complication is extremely dangerous for the patient. In order to suppress the reaction, the drugs that suppress the immune system,immunosuppressants are assigned.
  • Rejection reaction when the patient's immune system destroys the transplanted stem cells.

To prevent these reactions, the doctors of Turkish hospitals carefully select donor for allogeneic transplantation. The opportunity to use the close relatives - the patient’s brothers and sisters - as the donors are carefully checked. Pediatric patients have even higher chances as one of the parents can be a donor. Diagnostics before transplant

  • Pneumonography;

  • pregnancy test (for female donors).

Bone marrow transplant procedure in Turkey

The Turkish clinics conduct the transplantation at six phases:

  1. Preparation. A preliminary course of chemotherapy and radiation therapy for cancer patients to kill cancer cells of hematopoietic tissue. The course lasts for 7-10 days.
  2. Collection. The needle is placed under the ilium or femoral bones, collecting the bone marrow. The amount of seized material is 2,000 ml. Collection is conducted under anaesthesia
  3. Processing. From the obtained composition specialists delaminate healthy stem cells, which are frozen at -80°C. To reduce the risk of rejection, T-lymphocytes are extracted from it. Processing lasts for 1-2 days.
  4. Transplantation. The obtained composition is inserted to the recipient by intravenous injection. The procedure is similar to a blood transfusion and lasts for two hours.
  5. Neutropenia. This means the phase of adaptation of the stem cells in the body. In the period of neutropenia leukocytes in blood are practically absent, so the immunity is significantly impaired. To prevent infection, the patient is placed in a special sterile box and prescribed to take antibacterial and antifungal drugs. The duration of neutropenia lasts for 2-4 weeks.
  6. Engraftment. If the procedure is successful, the patient's immune system is gradually recovering. To monitor the recovery process at this phase, the Turkish doctors carry out daily tests of the blood, gradually stopping the antibiotic treatment.

Rehabilitation after transplant

The rehabilitation process is associated primarily with the restoration of the patient's immune system. During this period it is necessary for the patient to limit communication and appearance in public places. (S)he should also carefully maintain good personal hygiene.
